I'll try to sum up what the song is about:
He happened to overhear that she was hurt by someone, whose love for her changed and now she's sad. So now he wants to tell her what's in his heart, that he's someone who has a heart and who's waiting for her not far away. He asks himself how best to tell her, how to make her see... and sings "take my hand, don't look back/ what can I say to let you know you have me/ take my hand, don't look that way..."
